A tragic story. We've all seen photos of practices with players wearing the padded protective headgear on the outside of the helmet. Is there a reason it's not used in games?

I’ve read that wearing padding outside of the hard/slick shell (dramatically) increases the risk of catastrophic neck/spinal injures, due to the padding sticking/catching
